Camp Canine Doggie Daycare Agreement

I have read and understand the Daycare section of this website and/or the Doggie Daycare "Camp Canine" Information pamphlet. I understand the hours of operation of Ahwatukee Animal Care Hospital and Pet Resort. I understand that if I pick my pet up after my designated pick-up time, I will be responsible for extra charges as outlined in the information pamphlet and on the website.

If a pet is not picked up within ten (10) days after the designated date and time for discharge, the pet will be assumed to have been abandoned and will be put up for adoption or sold at the discretion of Ahwatukee Animal Care Hospital and Pet resort. Notice in writing of such action shall be deemed necessary. Any and all fees collected for the sale or adoption shall belong to Ahwatukee Animal Care Hospital and Pet Resort.

I understand that Ahwatukee Animal Care hospital and Pet resort Doggie Daycare "Camp Canine provides everything necessary for my pet's enjoyment and comfort during his or her stay at daycare. I am aware that the daycare is not responsible for personal items that be lost, chewed, damaged, or destroyed. (If you choose to bring personal belongings, please label them with the pet's first and last name prior to arrival).

If you choose to medicate your pet prior to arrival at the daycare, Ahwatukee Animal Care hospital and pet resort cannot be held responsible for any ill effects this may cause. We are pleased to administer any medications your pet may be required to take at a charge of $5.00 per occurrence (you supply your pet's medication).
